Filice understands the concerns Internet users have about the privacy and security of information that is collected on our website. We respect your privacy and we are committed to protecting it. This policy tells you how we do that.

Information Gathered by Filice

Filice gathers information about visitors to our website to improve the caliber of online information and services offered. There are two forms of data that Filice collects on our website: Site Visitorship Data and Personal Information Data. How we collect this information and what we do with it is discussed below.

Site Visitorship Data

Filice routinely gathers data on website activity. This information, “Site Visitorship Data,” is collected on an aggregate basis on public areas of our website, or on an individual basis on the password protected areas of our website that require website visitors to logon using an access code (e.g., userID and/or password). Site Visitorship Data may include information such as (i) how many people visit our website; (ii) the pages they visit and the features they use; (iii) how long they stay on our website; and (iv) the domain name of the Internet website from which visitors are connecting. Site Visitorship Data is generally gathered through the use of cookies and pixel tags.

A “cookie” is a small piece of information that an Internet site sends to a visitor’s computer. In some cases, it is stored on the visitor’s hard drive. Cookies can save the visitor’s name, access codes, screen preferences, the pages of the site viewed by the visitor, and the advertisements viewed or clicked by the visitor. A “pixel tag” is an invisible tag placed on certain pages of our website and is used to track an individual user’s activity on the password protected areas of our website. A pixel tag is also known as a “clear gif.”

Filice collects Personal Information Data such as name, address, phone number, e-mail address, etc. that website visitors provide when they complete any online form, application, survey, or when they request a quote for insurance.

Any information a website visitor submits to obtain an insurance quotation will be provided to a Filice agent if the visitor uses the online application to request an appointment with an agent. The agent may be a Filice employee or a non-employee agent.

Use of Information Collected by Filice

Information collected by Filice on our website may be used by us for a variety of purposes, including:

  • Editorial and feedback purposes
  • Marketing and promotional purposes
  • Statistical analysis of users’ behavior
  • Product development
  • Content improvement

Collection of Personal Information From Children

This website is not directed toward children. Filice does not knowingly collect or post information from children under the age of eighteen (18). Filice Insurance Agency Privacy Practices Notice


The purpose of this Privacy Practices Notice (Notice) is to inform you of the Filice Insurance Agency (Filice) Privacy Practices (available at We protect your and your employees’ personal information from disclosures that are not allowed by law or that are restricted or disallowed as detailed in this Notice. Filice gives this Notice as a service to all its valued customers and to comply with the requirements of the law.

This Notice describes how Filice collects, discloses, and protects the Nonpublic Personal Information (NPI) we gather about you and your employees. NPI includes any list, description, or grouping of consumers that is derived using any personally identifiable information that is not publicly identifiable. It includes the medical, financial, and character information that we gather in order to provide you and your employees with insurance, as well as your and your employee’s individual name(s) and address(es).

We may materially change our Privacy Practices and if we do, we will notify you.


1. Categories of NPI We Collect: We collect several types if NPI about your employees, including:

a. Information that we receive on applications and other forms;
b. Information about transactions with us, insurance providers and related third party administrators; and
c. Information we receive when you or your employees visit us through the Internet.

2. Categories of NPI We May Disclose: We do not share your NPI unless allowed by law.
In fact, our privacy policies are stricter than the law requires. The categories of NPI we share are only those required to implement and administer your insurance policy(s).

3. Categories of Affiliates and Nonaffiliated Third Parties to Whom We Disclose NPI:

a. Affiliates: The law allows us to share your NPI with affiliates. However, Filice does not disclose your NPI to any affiliates.
b. Nonaffiliated Third Parties: The law allows us to share your NPI with nonaffiliated third parties under certain circumstances. When it is lawful to do so we share your NPI with the following categories of nonaffiliated third parties:

i. Insurance Providers for the purpose of implementation and administration of your insurance policy(s);
ii. General Agents to be used in the implementation and administration of your insurance policy(s); and
iii. Third Party Administrators to be used in the implementation and administration of your insurance policy(s).

c. General Types of Businesses: The law allows us to share NPI with non-affiliated third parties whose only use will be in connection with the marketing of a product or a service. However, it is the policy of Filice to not share any NPI with third parties for such marketing purposes.

4. Former Customers: The law allows us to share the NPI of former customers. However, it is the policy of Filice to not share any NPI of former customers.

5. Opting Out of Disclosure to Nonaffiliated Third Parties: The law allows us to share NPI with nonaffiliated third parties for marketing purposes with your written authorization. However, it is the policy of Filice to not share your NPI with nonaffiliated third parties for any marketing purposes beyond those necessary to implement your insurance policy(s).

If you prefer that we not disclose NPI about you to nonaffiliated third parties, you may opt out of those disclosures that are identified in Section 3, Categories of Affiliates and Nonaffiliated Third Parties to Whom We Disclose NPI. However, if you opt out of these disclosures you might not be able to receive valuable information or service from nonaffiliated third-party companies with whom we do business. If you wish to Opt Out of disclosures to nonaffiliated third parties as described in this paragraph, please indicate so to your Filice Account Manager or Broker either by letter, fax, or email. In order to ensure that all opt-out requests are properly honored, we will only process written requests by letter, fax, or email. Your opt out will remain in effect until revoked in writing by you.

6. Disclosures Made of NPI Protected by the Federal Fair Credit Reporting Act: The law allows us to share non-transactional information you disclosed under the Fair Credit Reporting Act. However, it is the policy of Filice to not share this information.

7. How We Protect the Confidentiality and Security of NPI: We protect and safeguard your NPI. Filice maintains all the appropriate physical, electronic, and procedural safeguards to maintain the confidentiality and security of your NPI. We restrict access to NPI to those employees who need to know that information to provide products or services to you.

8. Your Right to Access, Copy, Review, and Request Correction of NPI: You have the right to access, copy review and request correction of any NPI in our possession. You must make this request to us in writing and we have 30 days to allow you to review your NPI. If you believe that there is an error in the information, you may request in writing that it be corrected. We have 30 days from receiving the request to make the correction or to inform you as to why we will not make the requested change and the reasons why. If you disagree with the refusal, you may supply us with a concise statement why you disagree and it will be filed with your NPI.

9. Disclosure of NPI Under Specific Exceptions: The law allows or requires us to disclose NPI in the following situations:

a. With your written authorization.
b. To a non-insurance entity if it is reasonably necessary for us to properly do our business and the other entity agrees not to disclose the NPI.
c. To an insurance entity if the disclosure helps the receiving party perform an insurance transaction for you or if it is reasonably necessary to detect or prevent criminal activity, fraud or misrepresentation in connection with an insurance transaction.
d. To a medical professional in order to verify coverage or benefits, conduct operations or service audits.
e. To the Department of Insurance, law enforcement, or other governmental entity including an administrative or court order, or as is otherwise required or permitted by law.
f. To conduct actuarial or research studies if there are proper safeguards.
g. To facilitate the sale of whole or part of an insurance business.
h. To a person whose only use will be for marketing a product or service. However please note:

i. No medical or character information may be disclosed.
ii. You may “opt out” of the disclosure.
iii. The person getting the information agrees to use it only for marketing purposes.

i. By a consumer reporting agency if the information does not go to an entity.
j. To a group policyholder to report claims experience or do an audit or to a certificate holder or policyholder to inform them of the status of an insurance transaction
k. To a professional peer review organization to review medical care.
l. To the government to determine eligibility for health benefits.
m. To a lien holder or any other having a legal interest in an insurance policy to the extent that the disclosure is needed to protect their interest.
